  The Mills County Wellness Coalition is working together to improve the health status of Mills County residents. We are gathering information to help determine what changes are needed to help people implement positive lifestyle changes that help them be healthier.

Your perspective as a Mills County resident, a Mills County Employer or an employee of a Mills County business is very valuable. We have developed a survey for each group and are asking your help in completing the appropriate surveys as well as passing this request on to others in the Mills County community. It is appropriate to complete all surveys that apply to you.
